Correct Order form?

PLEASE DISCARD ALL YOUR OLD FORMS! Use the current forms only.

The "County of Record" is always listed at top of the index list pages.

For Example:

"Steuben County, New York Surrogate's Records" = the county of record is Steuben.

Please do not send me any files with ".doc" extension. Save the order form as plain text, or ASCII and send it.

Orders for 7 or more record files will not be accepted. They will be returned unprocessed and uncashed.

Due to very serious problems involving Microsoft Office 2003 suite with either Windows 2000 or Windows XP, do NOT attempt to save as .doc under any circumstance. Save them as .txt (or ASCII)!!! Do NOT attempt to send attached order form to SAMPUBCO with file extension ".doc" under any circumstances for that reason alone.

You CAN highlight the form, copy and paste it in your email without any problem and fill in the form with ease and email to yourself if you are planning on mailing - OR - simply email the filled form to me if you are using PayPal, separate from the PayPal order. (PLEASE do not paste your order INSIDE the PayPal payment form.)

Please pay attention to information being required on order forms.

The records are not recorded in volumes. All loose pieces of documents are filed and filmed together.

Frequently each record contains 50 pieces or more of papers. Contains with any possibility . . . guardianship, original will, letter testamentary, estate sales, names of heirs with their addresses, etc. Even odd actions.
